Reliance - Security deposit not refunded. Following up from past 1 year

I had taken a connection in 2010, Feb. Due to poor connectivity at my house I disconnected the service on Feb, 2012. Since Feb, I am following up with Reliance communication customer care for the refund. Until now I haven't received the amount.

I am going file a case in Consumer court. Need help with the documents I need to submit as proof. I am not going to talk to customer care anymore.
